Starting a New Questionnaire

• After logging in, click My Questionnaire in the top left corner of the window
• The Questionnaire Review Page will have 3 sections
  • Start New Questionnaire
  • View Questionnaire in Progress
  • View Completed Questionnaires
• In the Start New Questionnaire section, click Start Questionnaire on the right side of that section
• You will be sent to the beginning of the questionnaire. From here you can start filling it out.
