Filling out Questionnaire

Select gender of patient. Click Next
Enter the age at diagnosis. Click Next
Enter the year of diagnosis (default is the current year). Click Next
Select one of the diagnosis' listed. Click Next
At this point in the questionnaire, you will select different options that are associated with the diagnosis you've previously selected.
