Create a New Account

• Click Create New Account
• Fill out Register Form
  • Opt In: This will save entered data and you will be able to save an incomplete questionnaire and return to view past diagnosis.
  • Opt Out: This will not save entered data and you not will be able to save an incomplete questionnaire and return to view past diagnosis. [*opting out will skip registration]
• Once the registration form is filled out, you will automatically be taken to the Welcome Page. From there you can start the questionnaire.
